TSA’s Building Blocks: The Sukh Initiative

TheSukh Initiative (2013–2019)stands as a cornerstone project for The Scaling-UpAccelerators (TSA), addressing critical barriers to family planning and reproductive healthservices in Pakistan. Supported byThe Aman Foundation,The Bill & Melinda GatesFoundation, andThe David and Lucile Packard Foundation, this initiative transformedaccess to contraception and empowered women in over one million peri-urban communitiesof Karachi, Sindh.

Key Achievements ofthe Sukh Initiative

  • Increased Modern Contraceptive Prevalence Rate (MCPR):Achieved a rise to54%, significantly improving family planning adoption.
  • Comprehensive Outreach:
    • Door-to-door services extended to one million individuals.
    • 189 Community Health Workers (CHWs) served 805,996 people, including140,309 married women of reproductive age (MWRAs).
    • 17,049 women accessed postpartum and post-abortion family planningservices.
    • Over 73,813 MWRAs adopted modern contraceptives.

Innovative Approaches and Sustain able Impact

  1. Family Health Days (FHDs):A game-changing concept piloted in Karachi, inspired by the Kasur Model, to provideaccessible family planning services at Family Welfare Centers.
    1. Over 1,791 FHDs served 25,810 clients during the Sukh Initiative.
    2. Theprogram’s success led to its expansion, serving 142,972 clients in Karachiand Hyderabad between March 2023 and February 2024.
  2. Life Skills-Based Education (LSBE):Integrated into Sindh’s school curriculum for Class 7, training over 500 teachers asmaster trainers, ensuring sustained youth education on reproductive health.
  3. Scaling Through Government Partnerships:
    1. Institutionalized interventions for long-term impact in collaboration withExpandNet.
    2. 22,000 Lady Health Workers (LHWs) trained to administer the first dose ofcontraceptive injections, and 3,000 trained in subcutaneous DMPA self-injection.

Catalyzing Change Through Participatory Processes

The Sukh Initiative successfully engaged government stakeholders, used adaptivemanagement for scalability, andleveraged participatory processes to ensure sustainableoutcomes.Today, the Sukh Initiative’s methodologies continue to influence public health policies inSindh, making it a model for scaling up impactful family planning and reproductive healthservices.
